Pre Surgery Examination

Before most forms of surgery, medical consultants are generally asked to assess pre operative risks in patients. This process is important in ensuring that your body is physically able and ready to undergo surgery. Pre-op is the time before your surgery. It means “before the operation.” During this time, you will meet with one of your doctors. This may be your surgeon or primary care doctor. This checkup usually needs to be done within the month before surgery. This gives your doctors time to treat any medical problems you may have before your surgery. During this visit, you will be asked about your health over the years, referring to your medical history. Your doctor will also perform a typical physical exam.

When performing this type of examination, there are many different areas that need to be taken into consideration. If issues are found in any of the following areas, action must be taken to either stabilize the issue or prolong the surgery:

  • Cardiovascular system – heart murmur. With the exception of emergency surgery, patients should be hemodynamically stable and their vital signs normal before starting anesthesia.
  • Respiratory system – abnormal breath sounds.
  • Gastrointestinal system – abdominal masses, previous scars.
  • Musculoskeletal system – skeletal malformations such as kyphoscoliosis.
  • General – local skin infection.

If results were found to be normal within four months of surgery and no change in the patient’s clinical status has occurred, no further laboratory test must be repeated. These investigations can be helpful to stratify risk, direct anesthetic choices, and guide postoperative management. The decision to order pre operative tests should be guided by the patient’s clinical history, comorbidities, and physical examination findings.

Patients with signs or symptoms of the active cardiovascular disease should be evaluated with appropriate testing, regardless of their preoperative status. Electrocardiography is recommended for patients undergoing high-risk surgery and those undergoing intermediate-risk surgery who have additional risk factors. Patients undergoing low-risk surgery do not require electrocardiography. Chest radiography is reasonable for patients at risk of postoperative pulmonary complications if the results would change perioperative management.
